Print 3 copies of this page. The transferer and transferee will each retain a copy. The third completed and signed copy should be sent by post to: Altek Instruments Ltd, 18 Fisher Close, Walton-on-Thames Surrey. United Kingdom KT12 5PN.

IMPORTANT: The signatures must be originals. Faxed or Photocopied signatures are not valid.
